Your member responsibility amount is due on the date of service.

VA Orders

Services are fully covered for veterans with purchase orders. We can submit the request on your behalf. 

Workers Compensation: UT, ID, MT, NV, WY
Services are fully covered for workers compensation claimants. Most WC insurances require pre-authorization. You can start the process by contacting your adjuster, or we can submit a request for service after an initial (free) consultation. 

If we are not enrolled with your insurance at this time, we will work with you and your insurance to have services pre-approved. We always have a self-pay discount option and can submit member reimbursement claims on your behalf.
